I would recommend an extra layer of fencing around your perimeter wall. I find prisoners are less inclined to try climbing to begin with if they don't feel it'll take them out of the prison in one go since they'll view the other side as just more prison. Another (and honestly better) option is to never allow them that close to the outer walls to begin with: that's how throw in contraband gets in anyway so you need to keep them away from the outermost walls.
Also if they're climbing perimeter walls regularly, that sounds like you got a contraband problem concerning rope. If it's bad enough, you may want to make the laundry staff only for awhile until you get things sorted out.
Yeah. Anywhere a prisoner has no business being should be marked staff only or the guards will have no reason not to let a prisoner in there if they want to be.
Though so early on you might also want at-least some solitary cells around the low and medium areas so they can serve out punishment for it before being transferred properly.
