Take the tools back to your cell and, with the tool in hand (has to be visible to show it's selected), right click on the toilet to start the tunnel. A little pop up should come up on the side of the screen telling you if right clicking will stash contraband, dig, or move in or out of a tunnel. Check that first before you click anything to be sure it's going to do what you want it to do.
Once in the tunnel, left click and hold at a tunnel edge to dig. Keep going until your tool either breaks or you arrive where you want. Right clicking on the tunnel while in it stashes contraband in the tunnel. Right click anywhere else will create an exit.
Guards won't see the tunnel entrance you made unless they actively see you create it. They will see any exits you make, even if they didn't see you create it. If they see you or anyone else coming or going from the tunnel the whole thing will get exposed, so you can get "ratted out" by another tunneler you didn't even know about.
Remember: pipes and open ground = you dig faster. Walls and foundation = you dig slower. Also most tool contraband is metal so beware of metal detectors. Even if you trip one though, if you can get to any cell, you stash your stuff (right click on a hiding space) before a guard searches you.
Dogs are only a threat if the prison has Periwinkle as a warden (the military guy with an eye patch.)
Also, for some odd reason, sometimes stashing contraband fails to work. If this happens you will need to drop your contraband and pick it up again quickly.
Actually most tools double as digging. Specifically any contraband with the tool label. Some are better at it than others, but I've yet to see a tool that couldn't also be used to dig.
Was surprised the first time I saw scissors being used to escape with.
Yeah, I've been going back and forth with a supermax prison design about whether or not to allow prisoners regular access to a library or an infirmary for drug rehab because of the scissors.
