Heroic on exmode is just more harder than insane.
Yeah I worked out you can start EX mode on a higher difficulty, as dagami84 says it is not clear, but can be done.
In doing so, I noticed I could start an EX mode on Heroic difficulty, but was not sure if this counted for the achievement, or a "pure" Heroic run was required.
According to FireStar sounds like a "pure" Heroic run is required, I'll give it a shot!
So unless you have a heroic completed playthrough stashed around from which to start an Ex mode on, you won't get the achievement at the end with a normal, hard, insane Ex mode changed into heroic difficulty in the menu.
Sample:
Start and finish a game on normal. => Achievement
Start EX-Mode and choose "hard" and finish it => Achievemt
Start EX-Mode and choose "Insane" and finish it => Achievement
And if i remeber correctly, Heroic Mode is same difficulty level as Insane but you can only choose it by starting a complete new game. So without any upgrades. Otherwise it would not be sooooo much fun hahahahaha

Just about to start my EX Insane run, then Heroic next.
I completely agree with the above comment. Nor do I know either.
Plus, there are so many games to play - how do you guys get time to replay a game fully?! Isn't playing a completely new game something that overrides replaying a game?
This is another area where I feel this game would really shine with co-op (hell, at least for survival mode). -insert comment about 2 wangs here- Gloriously brutal difficulty that makes you want to smash your monitor is a lot better with a good friend by your side :)

It is still an awesome game man :) Honestly, the primary issue for me on the harder difficulty settings is just spontaneously exploding. And I mean that literally, when you have full health and are sprinting/dodging as best as can be done and suddenly boom, instant death. Must be a combination of explosive damage. The thing is, the same exact thing happens in hard reset on the harder difficulties as well, so i think it is a bit of a design flaw. That is the main issue for me with the harder settings. I have been trying to play survival mode but around the 12-13th wave this same damn thing happens everytime. very frustrating :(
