I started on insane. Seemed fine really.

Other than the bosses just becoming more of a grind rather than more difficult, and bezerkers making things a nuisanse, it makes for a richer more eventful playthrough. There can/will be waves of enemies that might take a few attempts. Otherwise it's a smooth experience if you get the hang of what you should be doing. It's not absurd.

Also, you can save anytime (during a fight). This really eases things up and it's why there's a Heroic mode unlocked when you finish. That mode challenges you to finish a whole chapter without saves/checkpoints.

What Murray said, it might get pretty frustrating on the later levels though, where you are introduced to hordes of enemies along with the bigger ones.

Aight going balls deep then.
While playing it, it felt like the game would be more charming or characteristic by possing a challenge.

pretty frustrating and time demanding, especially the bosses cause you will spend a lot of time trying to kill them.
I played on hard my first time through. Kinda glad I did. Lot of stuff they don't really mention in the tutorial that I only really found out through loading screens. Also took a bit of time to realize that guns are pretty much pointless. The first boss also took nearly all my ammo.

So there is no tutorial / gameplay tips from hard and up?

there's a tutorial but they just fail to mention things. I didn't learn you could ADS in this game until a loading screen casually mentioned it on like lvl 15! Same thing with melee attacks while wielding a gun. It'd be like playing a tutorial for Super Mario Brothers and they just neglected to mention that you can hold B to run.

I just finished the game on Insane and as usual the highest difficulty ruins the experience, just like the COD single player campaigns. It's not too difficult, but enemies, even the simple mooks become bullet sponges, so guns are basically unusable. Be sure to upgrade everything that improves sword damage and use that

I'm playing on Hard. Chapter 16 in a courtyard with wave after wave of Berserkers, etc. I've probably died 50 times now. I don't even want to play anymore :/

I'm playing on the level just below insane because to be honest "Insane" usually kinda means it. But I'm finding anything but the katana or the smg practically useless. The revolver (which are usually my go to gun in every other game) Aims like a piece of ♥♥♥♥.
