Does the game have a New Game+?

I started playing it some weeks back till The Witcher 3 released. I had great fun and will continue playing this (and probably unlock all achievements)

But what I noticed that the game has an extensive skills and weapons upgrade tree that competes with RPGs.
so does this game have NG+? or can you carry on your progression when you start with lower difficulty? (because I am doing my current playthrough on Insane)

once u completed the game u can choose additional game modes,u can carry over ur progression if u want

I'm not sure if it carries between difficulties, but I know that you can do EX Mode on the same difficulty and it'll carry everything aside from your fully upgraded sword.

the weapon/power upgrades carries over all the way up to Insane Difficulty, if playing on Ex-Mode
