- Heroic mode(max difficulty and dying means restarting the whole level).
In theory you'll need at least three runs for all achievements, and then maybe some farming like killing with environmental damage only.
2. No, DLCs are only for sword models.
If you find the game to be hard, I would suggest starting a normal/hard (or hell, casual) playthrough, complete it, then do NG+ in Insane (as you will have all the weapons and upgrades from the other playthrough).
Keep in mind that the achivements that "depend" on a certain difficult (Do this on XXXX difficulty) can all be unlocked if you complete the achivement in the highest difficulty (so if you complete an achivement in Insane, the Casual, Normal and Hard variants of that achivement will be unlocked aswell).
the way to do this (providing you're not playing on Heroic), is to approach a fortune cookie and immediately save your game before picking it up. So that way, you can keep reloading the game over and over until the achievement unlocks.
yeah it's a bit of a cheat, but at the same time, it's sometimes best to get the collectible-based trophies out of the way first.
