edit: Okay, so you have to keep back tracking and such. The rules of teleportation are lame.
It seems likely it is a bug since playing through the game twice I never encountered a time when a teleporter door was open before I interacted with it.
I have since sent in my bug report and save game to Doublefine via email. Here is hope they can finally eradicate the teleportation bug (since it seems to have been an issue during playtesting as well that seemed fixed).
The doors of the red teleporter (both ways) were glitching out. They appeared open but when interacting with them, they would suddenly shut tight and I was caught in the conversation loop with the red teleporter even though I tried to teleport back (but it would ultimately not let me).
All four teleporter entrances refused access — except the fifth one, the blue return teleporter (near the air vent). After going through it the puzzle could be continued as intended. The red teleporter doors no longer glitched out and the conversation loop with the red teleporter broke. I could thus start the conversation puzzle with Mom after enlarging Vella's head twice.
It is probable that through some combination of teleporting back and forth I ended up using the blue teleporter last but didn't return through it which, for some reason, triggered the red teleporter loop and red door glitch down the line somewhere.
