Windows 10 FCU messed up Quakespasm
FCU means Fall Creators Update.

The game still runs fine in Quakespasm, only issue, it seems to have messed up the mouse movement, as every so often the camera will instantly change to a different position, which makes aiming a nightmare obviously.

You are running around, moving your mouse, then suddenly, instead of looking forward, you turn to face down, or up, or some other random direction. This happens quite often, and it is definitely quakespasm doing this, or at least the most recent version of quakespasm.

The modified version that's made for Arcane Dimensions doesn't have this issue at all. So this led me to thinking it's just the current version is incompatible perhaps.

Darkplace's lastest stable release, 11/21/16 works fine. Must be your engine. I recommend Darkplaces if you want good graphics support, along with EAX3 style occlusions

Yo this is definitely happening to me, and wasn't happening before the recent windows update. If you figure something out, let me know.

Yo, I uninstalled the FCU from System, Settings, updats and security, recovery(on the left, and then rolled back the update.

This fixed the issue with Quakespasm. Back to playing AD :)

I also have this issue but I don't want to uninstall the whole windows update to fix it. I am using Quakespasm.

Try using the latest build-test build. Try Windows compatilbilty. Or switch to Darkplaces/DirectQ or something

Same issue here, using Quakespasm 0.92.1 64-bit on Windows 10 after the FCU. It's a rather annoying bug. I still haven't figured a fix so far.

I think I've solved the issue by switching to the SDL2 version of the engine: use the quakespasm-sdl2.exe instead of quakespasm.exe

So I just tested this on the Quake HD Pack and Quake Epsilon Build, both of which have this mouselook issue (which makes the game impossible to play). Anyone hear of any fixes short of reverting the FCU update?

The texture pack in only usable with Darkplaces.
