Report a translation problem
Fire up SAM, look at the stats page for La-Mulana, and see which of the STA_OBJECTS_(insert number here) entries has a 0 as its value instead of a 1.
They're not in the same order as in the guides (which are ordered more by approachability), but if you tell me the number I can tell you which specific one the game thinks you're missing.
If you're not that desperate, keep trying. Consider making a checklist of some sort with two columns: one for scanning something and then a second one for when you've successfully saved since then. (That'll help deal with the scenario of "of course I scanned that! ... oh wait, then I died and didn't scan it next time.")
STA_OBJECTS_80 is the one I'm missing.
EDIT: Okay, scratch that. It's still not unlocking. Do you have to scan it before lighting up the door, by any chance?
I checked my notes though, and #80's definitely still that 9 symbol (the last one you move through to unlock the exit to that floor).
It could be that your save file already has credit for scanning it even though the achievement tracker doesn't, and the achievement tracker only updates credit when the local game data goes from "haven't scanned this" to "scanned this" (but it doesn't check it if the local game data goes from "scanned this" to "scanned this again").
Easiest way to test would be to run a new save through the early and midgame until you get that far. Then save beforehand, and try scanning it. I do know that achievement only cares about "have seen it ever" and doesn't care about doing all 131 of them in a single playthrough.
I had this problem too. My achievement went from 81/131 to 131/131 after scanning the rightmost Mushussu in D3. I guess the left one is bugged.
