Can't Hack

For TLDR, I try to hack a terminal and instead of minigame working, everytime I miss I take a crap ton of damage (I have Chroma Prime), but since no matter what I do I miss even when I don't, it downs me.

Are you sure that you weren't playing inside Nightmare missions that happen to have the "instant death whatever" text inside those before starting them?
It seems to be either that or you're playing an extremelly high level mission that has those settled into by default.
And on a related side note it's always useful to mod your Parazon[warframe.fandom.com] to increase the amount of time available to perform that console's hacking.

In some higher level missions they even disabled the "Y" key to instantly use a Cipher to bypass the hacking.
