Initial combo still lets you consume combo with heavy attacks, and it'll refill back to the 'initial' amount after doing so.
There's a handful of mods which provide it. Corrupt charge (gives 3x), Covert Lethality (2x- dagger specific iirc)... I think there's an aura mod out there. Rivens can also have it.
Certain incarnons have it, certain weapons have a passive, and there's the melee crescendo arcane now.
Initial Combo is handy for heavy attack spam builds, finisher builds, and with certain warframes using melee as a stat stick or whose abilities utilize combo count.
Since they have to 'refill' there's generally a limit for how much is useful when it comes to heavy attack spam. The 'melee crescendo is iNsAnE!!!' is clickbait for most cases of it sadly.
Other than that, what others have said you build combo count through regular melee hits at enemies. Then when you do a heavy attack, or slam iirc, you'll get bonus damage.
IF you have high heavy attack efficiency iirc, then you don't lose all your combo meter. Also some mods help you build up your combo count, while others stop your combo count from reducing so fast. (more time to build combo)
The higher the combo count the bigger the damage, but sometimes it depends on your play style, if it's worth building combo, and also the level of the enemies.

It's very good for the glaive weapons for instance, allowing you to constantly launch a heavy throw without having to make melee attacks first.
