As for the other two questions: no.
Let me just tell you that besides cosmetic and polarized slots (you can fill some builds without using Forma) they are exacly the same, with also the Prime version having more energy stock.
Slow down and take your time to read the article. There's no way you actually did read the Orokin Void article with all the reward tables in the two minutes it took you to post three times. I'm not writing you another big article. As for the other stuff: the prime weapons and Frames work the same in the Foundry as the normal ones.
You cannot directly upgrade your Loki to a Loki Prime. You will need to make a separate Loki Prime.
You find the blueprint, helmet, chassis, and systems for each frame, and various parts for weapons.
It's all random. You run the missions and hope you get them. To see where the parts are, look up the frame or weapon and check the drop zones. Try looking for Orokin Void in the Wiki to find out more.
Also, yeah, reading and research skills mate, gotta work on those.
The name?
Prime 'frames are otherwise built like normal ones; get the blueprints for the chassis, systems and helmet, build those components, then get the blueprint for the 'frame itself which combines the components into a useable 'frame.
