- switch to full melee mode and press your heavy-attack button
- equip and switch to a single-wielded pistol and hold your quick-melee button
So will have to see what happens with the next 'throw' melee weapon I come across.

The sword+shield combos are special, it depends on your stance. Try holding right-click and then pressing melee.
If you want a proper thrown melee I think glaives are a better option.
Glaive weapons can be thrown with a grounded heavy attack or by holding the melee button when you're dual wielding it with a one handed secondary.
The Wolf Sledge can only be thrown with a grounded heavy attack.
>>The Sigma & Octantis (which I imagine is the Sword and Shield you are referring to) is thrown when you do a melee attack in the air.
