Some questions about obtaining vengeful charge from lich

Here is what i think i should do: To obtain the charge effect, i need to kill or capture my lich. To do so, i must equip my requiem mods in a specific sequence and then kill/capture.

What i do not know: -There is a column on the right side which shows the number of attempts, does it have a max limit? if so, then what will happen after i reach my max attempts?

-Requiem mods have 3 charges. How are these charges used? Will a charge be spent if my mod is in an incorrect slot? Does the number of attempts have any effects on mod charges?

-Is there no other way to get the correct sequence other than guessing?

1. You have to kill the lich to get Ephemera. You won't get it if you convert it.

2. Number of attempts is infinite. It is simply a list to show you what you have already tried.

3. Charges are only used upon using the full correct combination

4. The more Lich Thralls you kill and the more you fight the lich the more requiem mods used in the combination will be revealed. After that you only need to guess the order.

Incorrect. You get the ephemera from both killing or converting it. You only get the weapon on kill.

Gah, I even went and checked cause I wasn't sure and the only source I found says it's kill only. ;_;
