Report a translation problem
resetting my controller to its default configuration fixed this, but it also assigned melee channeling to the R3 button (right analog click along with secondary weapon fire)..
Unfortunately that didn't work for me. Still only a single melee swing and then I'm stuck with my ♥♥♥♥ in my hand until I swap back to my ranged weapons. This has been extremely frustrating for me, I'm really debating just dropping the game and coming back to it in a few months to see if this junk is sorted out. I just want to swing my hammer into some infested's face without getting diablo style click induced tendonitits, doesn't seem like much to ask to me.

I was having the same problem. I fixed this by going into the game settings and setting "Melee with Fire Weapon Input" to on. Now it lets me use my trigger button when in melee without any issues.
