I guess this is why the changes to stealth kills before this patch came in to prepare for the melee update, makes it a real pain now though as anything that can't disable a whole troop of mobs can't get stealth kills anymore with guns and melee.
I'm sorry, but you're wrong. You can still switch between melee and ranged as before by using the melee button or the aim button. And once you've pressed your melee button and thus are in melee mode, you can use a heavy attack. It might only be possible if you have at one hit on your combo counter, though - I haven't really tested that part. What's new, or rather what's returned from ye olden days, is that you can hold the weapon swap button down to go into melee only mode where you can block manually.
There are some other changes that aren't pertinent to this discussion.
