Go to the game folder, rename MOUSE.CFG's extension to ".txt", edit the bindings, save and put the extension back to ".CFG". For the codes of each key refer to the "Scan Code Dec" column from this website: https://msdn.microsoft.com/en-us/library/aa299374

Or just run SETUP via DOSBOX by adding SETUP to the Autoexec.cfg part of the dosbox config, then you don't even need to look up the key scan codes, you'll get the full SETUP that DOOM shipped with.
