That was added by users. Steam doesn't do a great job of disambiguating between developer and user applied tags, which is why you will sometimes see strange tags on games. For example the Dark Souls games are frequently tagged as "dating simulators", "casual" or "relaxing" by Steam users as a running joke.
That being said Clown Meat does have some gay/trans characters, the game's narrative isn't about these characters but they are part of the supporting cast.
Additionally that tag may have been added due to the core development team (myself and Autumn West) and musician (Hannah Monsanto) of Clown Meat all being trans women.
So I suppose in a word to answer your question, "No" Clown Meat isn't a gay game or about LGBT issues, its just a game that has a few gay characters.
