How to Counter-Maneuver?

Having some difficulty for that quick time event to counter-maneuver, mostly I just don't understand how to angle the plane to allow me to do it. When does the icon for the counter-maneuver appear, at like which angle?

Well, first, of course, the green and red arrows need to be aligned.
Then you need to level your plane, meaning basically fly directly ahead, not too much banked/rolled. Then, you should be able to counter.

Having same issue. I can't even find the way to make those 2 arrows to appear!

When an enemy has initiated DFM on you, you should see the green and red arrows directly below your HUD. If they don't, the enemy plane is either not in DFM mode or still too far away, which also means he can't effectively shoot at you.

When those arrows appear, go full brake and level out your plane. As soon as the arrows are on top of each other, press whatever it is you press for the DFM mode.
(I use the Xbox layout for my 12-Button dual analog controller, so for me it's LB+RB)

It's about your (and the enemy's) speed rather than the angle. You can only do the manuever when the plane is leveled (flying perfectly horizontally), and flying pretty close to the enemy behind you, but to get the two arrows to align to let you do it you must find the correct speed (try breaking and boosting and see which makes the arrows move closer) while avoiding the enemy's targeting reticule.
