You'll see that 20 CHA gives you the best prices for items you buy and improve reaction of people you meet in-game. But it doesn't change the alignment system, thus evil characters will still leave your party at 19 and 20 reputation.
Plus, the reputation you lose through random killing goes up with how high your reputation is, so despite how many awesome things you do, you still can't get away with more than one murder at a time.
It would work better if there were two meters, one for good and one for evil, and of course if one outweighed the other too much... well, you get the idea.
BG1:NPC Pack has an option that allows you to pay various shady bards to spread your exploits to the masses in a "darker light", which is basically reverse donating.
Tweaks Anthology has a couple of useful options to look at; first is a merely cosmetic tweak that allows you to hear the neutral NPC "happy sounds" when your Rep is well ... neutral (Apparently the sounds exist but were never activated.) The second changes how the NPCs react to your Rep; one of the options allows the NPCs to become angry about your Rep but they won't actually leave.
