It was always there. However, it is easily solved:

A. Kill the unwanted companion
or
B. Lock the unwanted companion in a room where you'll never return and dismiss them from your group.

Your memory notwithstanding the paired NPCs have always been there. You can find 20 year old forum threads[sorcerers.net] discussing the situation so I doubt that we will be able to discover any new answers here. [Edit to add: thank you for making me feel old]

Given that BG2 does not have any paired NPCs the developers presumably agree with your opinion that paired companions were a bad idea.

With Montaron and Xzar, I simply tell Montaron that we've got his back and send him in 'alone' to fight a mob of enemies. Voila! Problem taken care of and Xzar is none the wiser. He even mocks Montaron for dying. Yeah, the dude is evil but I greatly prefer him over Montaron.

As far as the pairs go:
Khalid and Jaheira
Xzar and Montaron
Minsc and Dynaheir
Eldoth and Skie

To get Minsc without Dynaheir -- Go to the Gnoll Stronghold before you take Minsc into your party and rescue Dynaheir. But when she offers to join you, politely decline, She will leave in a huff, and you can go get Minsc and Boo.

Another thing you can do to have Minsc but not Dynaheir is to do Edwins quest to kill her first and then talk to Minsc :D

Thanks for information. I wanted an easier way to get Minsc into the party.

Just send party member into a group of enemies to die and then kick them out after they dead, problem solved.
