When you have that selected, and started the game, open console and you'll see game plays file with .tim extension, while FM synth options will play .mus files - these emulate OPL2/3 synth (like the Yamaha chips on Soundblaster family of soundcards).
Strictly speaking, there's no Microsoft General MIDI synth option at all in this rerelease, you could say that "Original" option is one because Microsoft General MIDI was based off Roland family of synth, but not exactly the SC-55 model used by Bob Prince for Doom.
So technically if you select "Original" you hear music (played as .ogg files) from games included here: Doom, Doom 2, Final Doom, NRFTL, Sigil and Legacy of Rust as you'd hear them played via "MS General MIDI" (roughly) yet with other music files (other wads) you'd hear music synthesized via "fallback" options: GUS emulation or OPL FM synth.
Yeah and without it, some wads music sounds broken, off key, or are missing instruments only present in that option. Modders worked with what they had and made some pretty damn good stuff even it didn't sound true to all the 5 people using the external hardware.
Other ports kept it in there as a fallback for this exact reason. Now Kex crew thinks they're too good for it? You cannot just rewrite history, a lot of wads are not going to sound right without it. Hell, it's the reason I still use other source ports over this when it comes to wads from the community.
