I don't really need any settings other than my framerate, video resolution, and the usual control settings, which this port has. I've not had any problems with choppy movement or mouse stuttering, and I don't even remember (or care to check) if I have vsync on or off, but I've not personally noticed any huge delay. Then again, I mostly use a Switch Pro Controller, but when I do use the mouse, I haven't really noticed anything out of the ordinary.
Sorry to hear you're having problems like that, though. Hope it can be fixed.
The problem is there, there are those who notice it and there are those who don't and are exclusively of Unity port. You just have to try the other source ports to understand.
Maybe it's just too subtle for me to notice, maybe I'm just not having that problem, either way, the game feels just fine to me. I've played numerous ports and source ports (OG xbox, xbox 360, chocolate doom, skulltag, gzdoom), and it feels pretty much the same to me, personally. I dunno, I just haven't really had any problems with movement or aiming feeling off.

The retail release is "good enough" for me.
I really wish they'd allow for a fully-expanded, no-HUD experience, though.
It's a bit blasphemous that this is missing as an option, but prominently displayed in the demos.
(the 5.1 surround sound from D3BFG would be nice, but not an utter necessity like the viewing area)
