Immortal Achievement Trouble

[Northern Circuit]

It says to die at least once to revive and escape dream hell.
Well, I'm in Hell and I use the Elixir, but I just keep dying with no revive.
Whether it's an instant kill or just hits me a bunch, I just keep dying.
I had it happen once, that I had 5 health instead of 10 and I thought that was the revive, but then I die before leaving anyways.

What am I missing? I don't get a prompt for revive after death like with Saiko.

NEVERMIND Turns out you often die by instant kill cutscene to Elissu but if you drink the elixir and then eat a poison mushroom instead, it will kill you and give you the revive prompt.
Now I just gotta get all the achievements in the first saiko ... whoo hoo
