2. I had this question too, but after playing a little longer, I think its just certain gear is under yellow, green, or red. When you choose which one to upgrade it'll show at bottom which gear you have will be effected. As you level up the same color, the benefits lessen, but can still be worthwhile, because a 10% damage and defence bonus to red when you have red weapon and armors is better than a 40% upgrade to yellow when you have no yellow items, but it could still be beneficial to choose yellow since it'll upgrade your overall health 40%.
3) I average around 45ish minutes for a run. I just unlocked the hardest difficulty, so that time may increase a little, but I wouldn't say it'll be much more than an hour or so for a run. At that point, I think you'll just be rerunning and trying new builds as you earn money to get everything in the shop.
