Oh yes!!! Same thing happened to me my first play through That note is easy to miss!
Though I did have to resort to a guide telling me that the note on the wall in that room could be examined as I spent an embarrassingly long time trying to figure out the code based off the drawn code you could just barley see by moving the camera around.
The other time I resorted to a guide was due to the face palming realisation near the beginning of the game that I ended up missing out on finding the pistol cause I didn't put see the code on the wall and I ended up using the axe all the way till I got to the shotgun room and started to suspect I may have missed something.
That was when I realised I have really been spoiled by the massive amount of hand holding from modern games.

and then on release say kobe~
First time I threw a grenade through the hole it did not blow the wall so I spent more than an hour trying to find another way or solution. I finally got it by throwing a grenade in the hole at an angle from the left so it lands more behind the wall.
