Ctrl.R probably means your right control-key (next to the arrow keys). At least that's how I would understand it without any context.

in the way that you say it it probably means the tight ctrl key, if doesn't then it means you have to press the ctrl key and the R key

Yep, it's the right-hand side CTRL key on your keyboard...

^ what the guy above said, this game is also very playable without a controller as well. They did a very good job with the key settings for controling the 2 brothers.
