I think I know where the problem is:
Raw Stone is NOT stored in crates, If you build a crate in a stockpile square where stone is already stocked stone will not fit in and become unstocked on top of the stockpile IF there is no free square inside that stockpile.
So If you build a lot of crates at once (around 8) the unstocked stone that took the plce where the stone was before the create was build will be enough to create a golem by chance.

It's before i put crate's, usually half-way through the first year, after i have the gnome's clean up everything and put it on the ground in the stockpile. after that it's fine, doesn't happen again.
which is why i'm starting to think it's spawning them from the dirt mined out to create the room, but since it's so early they don't spawn untill the "grace period" is up (ever notice the first half of your first year nothing spawn's and no goblin's ever come) and the fact that i've put a stockpile over it doesn't matter because the floor hasn't been "cleaned".
I believe golem's aren't able to spawn from recorces gatherd and placed in a stockpile, forgot which patch this was...
Opting into the indev branch fixes this, but you may end up with other errors instead. Outside of doing that if it's a problem consider using a trader to sell all your dirt / stone since it will also sell any unreachable clumps on your map.
That's false.
Golems spawn based around there being enough material close by. My mine is lit up 100% and they can still spawn if I leave enough raw stone around. They even seem to ignore normal enemy spawn rules in the current version. Normally a gnome needs to be at least 16 blocks or up to 2 floors nearby for enemies to spawn. That's how skeletons, beetles, zombies, and spiders work, but they need it to be dark. I've had golems spawn at depth -50 because the distance to haul the raw stone was farther than other jobs.
Also golems can spawn on the surface, dirt golems in particular I've gotten many of since I'm flattening the surface now. Not only have they spawned from my stockpiles, but it also didn't take 4 days for them to spawn from a mass of dirt.
Bummer. I wanted to play with automotons and start a new world, but the experimental world I have now has everything but goblins and mants disabled. (dont ask, i am doing science)
Raw Stone, Dirt, and Clay isn't stored in crates. If it's a big issue consider selling it to traders or have gnomes dedicated to prospecting so they keep your stocks under control.
EDIT: Or just place a training ground nearby and have a squad train there.
ahh didn't know they weren't stored in crate's, thought everything "solid" was stored in crates... Considering trading it, i wish raw stone was worth something, hate giving stuff away in this game considering how much work you put into managing your kingdom, and how usefull it is to have around (decorate/furniture/replacing wall's/statuette). And It's not so much an issue now that my gnomes are fully trained (just really annoying having golem's pop up inside the base) but i will keep this in mind for future playthrough's.
I think it's still worth it, you just have to figure out how to work around it that fit's your build style. i think i'm going to have a bunch of "small" stockpile's (small enough so golem's can't spawn). And auto's are fun but they can't really fight (they don't equip armor or weapon's) so i make a squad and just have it set to hauling plus w/e i need done atm.
Stone slabs are stored in crates and worth 1 trading value. That's another option to just have stone cutters turning it into slabs. Which as far as I know don't spawn golems.
