As for the fight itself, try to lure him to one of the four corners when you think he's about to appear. If he fakes appearing twice in a row (when he clumps together), you can be certain that he'll reform next time. Personally, I like to stay near to him when I think he's going to form, as it makes it much less likely that I'll miss. Still, the window for hitting him is rather small. And in addition to the obvious issue of attacking him too late, sometimes, if you try to attack too early, you may find yourself passing right through him even then.
So yeah, you really need to work on the timing for this boss. Additionally, you may want to find and equip the Silver Shield from the Shrine for some additional DEF.
