Also have you restarted since you turned game flight mode off? In 2.9, at least for me, it will occasionally glitch and act as if it is on even with the box unticked when I start the game up again, requiring me to enable and then re-disable it to get aircraft to function correctly. You might possibly be having this issue.
Though ultimately you'd be better off posting on eagle dynamics own forums and uploading a track file of an attempted takeoff, the guys there can go through it frame by frame to see if any control oddities are happening.
Is your stick in the cockpit actually moving when you move yours ?
Are the control surfaces of the plane moving (rudder, ailerons, elevators) ?

To me the tests I'd do would be
- Try with a mission where you already are flying (can you go up?)
IF not... Check key/axis binding
IF it's OK
- Try with a hot start directly from the runway with minimal weight (at least no missiles/bomb/rockets equiped)
IF not working: Check throttle axis (is it really 100%)
IF working: I guess you forgot something on cold start
Should make it easier to troubleshoot after that

Free flight is ok, so axis are supposedly OK
My guess would be either overloaded (100% Fuel + wax heavy full weapons ... espeially if you forget flaps)
Or an accelaration issue (throttle axis)
What is you max speed when attempting to take off?
What about your payload?
I did do the INSTANT ACTION - LANDING mission and landed OK. I then went around to the tack off point on the ground and the aircraft took off with no problem.
I might add I had the same issue doing the Training Take off with the guy telling me exactly what to do. Needed to do it as I've been away from DCS for a long time and forgotten more than I ever knew.
Jel, Yes it was a hot start the mission is from the INSTANT ACTION and that is how it starts.
Yes the joystick, thrust pedals and all other instruments are working OK.
Grey Face, Yes to everything you suggested. I have doubled up again to be sure.
Grüntherspöth, I did all you suggested, the actual speed looked to be the same as showing in the YouTube video posted above by Troll Norris.

Xupicor, thanks for the input at least it shows it's not a bug and it is something my end.
The brake pedals are working OK they hold the aircraft with the engine revving at high.
The Force feedback in misc options was checked so I unchecked it. :)
Also, maybe click game mode and click again to uncheck it. Just something someone recently did to fix their issues. (Game mode off doesn't always stick, looks like?)
Please post a track when it happens again.
