Chaff modelling might not be the best but generally countermeasures aren't like in movies or Battlefield games were you just pop some chaff and flares and the missiles home into them with 100% success ratio.
You need to actively maneuver to avoid missiles and drain their kinetic energy while hoping fingers crossed that the countermeasures do their job.
Maybe the missle and you were head to head....after you release chaff....you have to be like 90 degrees appox to the missle.
Head to head with any missle and still using chaff or flare is a sure case of disaster.
