been a long time since i've played and i am currently going through the game again, but i'm fairly certain it ends on 7 because you never end up fighting sick simon, but you fight book simon instead and sophie survives and simon accepts the doctors treatment and never ends up commiting suicide and finds acceptance and some relative solace and peace while finishing his book.
the game itself is you following sick simon's own writing, where he writes about himself and the trauma and the pain and depression he is in, but the player or "book" simon, is the one we follow in the story, but your choices dictate if sick simon recovers from his torment and accepts treatment, or not.
in chapter 8 "book simon" ends up fighting the real simon aka sick simon, this ends up convincing him to kill himself, but the entirety of chapter 8 is about him killing himself which if you get ending four, never happens so he never writes that chapter unless you get the bad endings.
the fourth ending is the canon ending because its probrably the closest simon gets to a happy end, which is what most people wanted for simon, he accepts the situation he is in, he ends up trusting the doctor and allows himself to be treated by him and sophie ends up visiting simon regurarily at the mental hospital and simon survives although stuck in a mental hospital for the rest of his life for killing the two officers checking up on him during his psychosis instead of himself.
