The Story of Cry Of fear explained!!
In the first cutscene of the game, Simon is hit by a car as he asks a man who is crawling on the ground what had happend to him. This car crash had crippled Simon into a wheelchair, unable to move by using his legs. This caused him to feel depressed and isolated because of his lost friends after the crash. Simon's mentor and therapist is Doctor Purnell plays a HUGE role in Cry Of fear's story. He is portayed as the bad guy in the game but it is proven in cutscenes that he is just trying to help him. When you kill Dr. Purnell in the attic a flashback appears and it shows him asking Simon to write a book about how he feels. When Simon writes that book, Cry of fear begins. Simon said himself that his life is a never ending loop of anxiety and despair. The car crash earlier was the final push to insanity mainly because of his anxiety. That is why the game starts there, because when the game starts the book begins to be written by simon. After the car crash Simon wakes up in an alley which symbolizes how lost Simon is every day when he is depressed. All of the monsters in this game a either crippled, defected or disembodied, Simon feels fear, danger and damage. The monsters are Simon's creations! Every monster, every boss fight, every second, every experience you had in game was all happening is Simon's book. The Sawer boss for example has nails and cuts on his neck symbolizing self harm which is what Simon feels like. Not only that, after defeating the Sawer it cuts it's own head of symbolizing Simon's want for suicide. Another thing you should pay attention to is the fact that if the monsters are Simon's creations, killing them is Simon's recovery. If you die in game, Simon has commited suicide in real life because of (for example) the Croucher monster, the Croucher's low stature could relate to Simon's difference, or low status to the rest of the world, having no legs to meet the heights his eyes would normally see prior to the accident. Another relevance being the Croucher's function as an enemy to attack Simon's legs, harming what he lost in the accident which had killed him. The Mace boss has bandages all over him and it's slow. This could symbolize Simon's injuries. Also with the bandages over it's eyes could symbolize that Simon is feeling blind and depressed. The Carcass boss cutscene involves Simon and Sophie having an emotional chat, and Simon relizes that Sophie does not want to start a relationship with him or want to be friends with him. This destorys Simon and puts him into a depressed state. Simon loves Sophie and Simon watching her leave him behind and suicide makes him life worse. The Carcass boss is a piece of meat with amputated limbs sitting in a chair, this relates to Simon because he says that he feels like a piece of meat sitting in a chair. If you flee the carcass boss, then he never comprimised with sophie and Simon is so obsessed with her he murders her and then take her with you into an alternative life. This triggers a bad ending. If with fight the carcass you with Sophie and she stays alive until the end. In the Asylum, if you give the doctor the gun it shows that you trust him but it hurts you in the illusion of the fact that he shot you. if you kept the gun then killed him in the attic, in the non book world you killed him in his apartment . Every thing that happens in the game, it is Simon writing that book. In Doctor mode, your objective is to find Simon's book and destory it. Why you may ask. Well it is because all of the events in the campaign are making Simon feel worse so hes trying to save him.

You also left out the fact that simons book does have some hold on the real world otherwise the doctor and the policemen wouldn’t have fought the monsters on the way to simons apartment

Than again umm well doctor mode and the cop campaign are more what if scenarios so maybe it doesn’t Idk that always confused me if Simon was having a psychosis than how was the doctor there in doctor mode

ik, but simons goal in the book is just to get home and call the police
Umm I don’t think police were really part of the plot that was just cause he was attacked but after a while he kinda gives up on it
