This makes no sense - what if I map healing potions to hotkey? If the same things happens, I'd be ♥♥♥♥♥♥ in the middle of a fight, trying to replenish, but find my hotkey isn't working???
Since the torch is about the only item with a limited use, nobody has bothered to fix this bug. It is just easier to hotkey it to a Nighteye potion or play a character that has Nighteye like a Kajit.
When I first started my current character, I was having the same problem, but with 3 being bound to the body armor I was using, so I could easily unequip it to run faster. For some reason it kept getting unbound. At the time I thought it had to do with having a second one in my inventory and it became unbound or because I put the one I was wearing in a container later to use the newer one so I didn't have to repair it.
I think it becomes unbound when the item you bind is no longer in your inventory. In the case of torches, the game only recognizes the torch you had bound and when that one is gone the bind disappears.
You can probably test it by using a torch with the bind and jumping in a lake. When the torch goes out check to see if your bind is still there.
