Is there a way to hide this? I'm using xobse, 3 engine bugfix mods, and ORC, and Maskers OB Overhaul. Only appears after shift tab.

ORC borderless is enabled. Launcher option is set to windowed. Do I have to do something else? How to tell if OB is actually in borderless mode?

Use either Escape key (to bring up option) or use Journal both will put game in UI mode.
For me this have reduced crashing due to ALT+TABing which I do a lot during modding and to check quests etc.

Always put UI (Journal/Escape) up before ALT+TAB and then you should be good to go.

The game isn't crashing much for me- I think 3 times in ~50 hrs- but the spinning blue circle really interferes with playing the game as it's like a second crosshair that moves relative to where the windows mouse pointer is. Restarting the game gets rid of it. There has got to be a way to hide it...
