Don't forget to read the Notes and Bugs sections carefully (and make sure you have the Unofficial Oblivion Patch installed).

And several bugs of the Sanguine quest in particular (read the uesp description of this quest).
If you don't use it, you'll have to live with the bugs (or perhaps use the console? - although I wonder why someone would use the console, but not the UOP).
But consider using the UOP (read the description first, of course). It's the only patch / mod I always use (plus the one for Shivering Isles when I play this expansion). It's not absolutely necessary (as far as I know, you can complete the main quest without - I never tried it myself), but saves trouble and frustration in many areas and quests.
Any particular reason why you don't want to use the UOP? I'm just curious.
One time when I had a problem with another quest I was looking at one of the forums and it said that the patch fixed 1 bug but added a new one. I think it can be with other quests as well.
I'm not sure, but it seems that the patch changes the text of some dialogues, I want to read the original texts as much as possible.
Well, without the UOP you're very likely to run into problems with quests - don't be surprised.
On the other hand, you can always visit uesp when you have a problem with a quest, bug or not. And the UOP is still there as a very last resort before giving up (if uesp says the UOP fixes a problem you can't solve). And you don't have to complete all quests. Not even the main quest - I spend much more time playing Oblivion without ever touching the main quest than playing the main quest.
