That's why I think I remember that the readme recommends never throw something unique or that you wouldn't want to lose!
I'd bet the actual item fell through the floor into the void and the model stuck in the NPC is just cosmetic (i.e. not actually there) and that's why the retrieval function is not working.
If it's a standard base item, just add a new copy using the console with:
You'll just need to lookup the <code> needed for that.
EDIT: Keep in mind that the same thing can happen with the vanilla game "disarm" ability, i.e. if you get disarmed by an NPC, your weapon *can* and often will fall through the world. It's the finicky Oblivion physics engine implementation that causes the issues in the first place.
Just be prepared to save often!
If it happens a lot, i would remove it.
If it does not, do as Leeux suggests and when it happens use the console to add your weapon back.
