At some point I had a mod that basically provided the heightmaps for the whole world, but without any trees / cities / NPCs it was pretty dull to explore.
In terms of other lands, Midas Magic has a star gate which leads to quite a few. They're not especially large worlds, but there's plenty of variety.
I used to spend hours doing this and trawling each category.
Knights Of The Nine: Revelation adds new lands but you need the original Knights Of The Nine DLC. Excellent mod and very atmospheric.
