Does Oscuro's Oblivion Overhaul require a new game?
The mod page, Wikia and the Readme repeatedly remind you to start a new game if updating the mod from a previous version, but not once do they tell if newcomers to this mod need to start a new game.

Since it is a game overhaul and changes a lot of in-game stuff I would say that you need to start a new game in order to use it.

No need to start new character to play OOO

Actually one of my main character started play with OOO after doing many Quests
I did not encounter any problems and i even adds new Quest line created by me that edits some OOO scripts around Necturnal Statue(Shirne)
They works together completely well.

Thanks for the answer. I am currently at level 3 and have only completed 2 or 3 of the main quests..

I will try out the mod when I play next time. Hopefully I won't forget and I will report the results here, in case someone else curious bumps into this thread.

How did it go? I was looking forward to installing this overhaul but on an existing save with a charackter above lvl 20.

You can install OOO on an existing save, but it's advised to wait 3 days respawn time in a non-connected cell (like Testing Hall) before resuming play.

Also, if you use Wrye Bash, you also should use the Relevel NPCs function from the Save Game context menu as that would apply correct levels to persistent characters that are buffed by OOO, else they would be a lot weaker in your game (compared to enemies, that is...)

If you don't use Wrye Bash, well... maybe you should! ;-)
