There is this old method, enabling/disabling sound on the ini file, that I initially disreagrded since it seemed absurd.
In desparation, I tried it, and behold! No more stuttering or low FPS!
Nothing else worked, but that, which I did only in desperation

TL:DR version: The game was fine 2 years ago before I stopped playing after my cat died. This week, I spent several evenings troubleshooting this, checking mod files, editing ini files. I realized the stutter happens when the music changes, and with certain action sounds. I re-checked my audio drivers. Then I uninstalled anything audio related that I installed after I stopped playing 2 years ago. Each time I made a change, I ran the game to see if the stutter went away. I uninstalled DivX, and the problem stopped.
