Easiest way to do that is to go to the nearest Temple. There's one in cities like Ald-Ruhn, Balmora, Vivec. go to one the shrines in the temple and at least one of them has the option to restore all your attributes.

One that will have it is a Shrine of St. Veloth, and the blessing would be Almsivi Restoration. You could also find an Imperial Cult Altar, those are not so straightforward to find. There's usually one in every fort in Imperial towns like Pelagiad, Ebonheart, and Caldera or the larger imperial forts outside of towns like the ones outside of Balmora and Ald-Ruhn. An easy one to find is one inside the Mages Guild in Ald-Ruhn. It has a Restore Attributes option.

The other Dunmer Temple shrine that has Almsivi Restoration is the Shrine of the Tribunal.

In addition, if you have an attribute that has been damaged but are wearing a constant effect enchantment item that pushes it above your natural level, you must remove that item first otherwise it won't restore you properly.
