It's pretty straightforward... just download it from Nexus (pretty sure you'll need to make an account for that though) and then run the installer... then you'll have 3 options... either make everyone have invisible underwear, or make everyone be naked underneath, or just have everyone naked at all times

It's a good mod, really improves the ugly bodies vanilla morrowind has, although I'm not sure why anyone would choose the "make everyone naked" option

Sex appeal aside this is a good mod to have as a good number of equipment mods use the improved bodies.
