Either that, or you're doing this sequence out of order. The way it's intended that you do this is that you go after Dagoth Ur first, THEN go after the Heart of Lorkhan.
The bug can be traced back to morrowind rebirth, which breaks the final quest this way. Doesn't matter your order, you can kill dagoth ur first or go straight for the heart, rebirth will break the game. Without bugs, the proper order is to simply go after the heart, fighting dagoth ur is optional.
Mgso also breaks the game, but not in this specific instance
