S Ranks and P ranks

I have a question
If I get all the s ranks in for example world 1 before the boss
and change them to p ranks before the boss
and then defeat the boss
do I get both achievements or only 1?

These are separate things indeed, you have achievements both for S and for P. If you got P on them all from the get go you also get the S one by the way

So basically, if I P rank a world, I automatically get the p rank and s rank achievement for that world?

Yes! Happens with me when I have one level left to P Rank in a floor and I do accordingly so

Nope, plus they transcend save files, meaning the unlocked clothes will be there for a fresh save
