But if he meant skirmish then yeah, the game AI only have 3 difficulties: easy, medium and brutal. It was Red Alert 3 (and C&C3) that gave four AI difficulty: easy, medium, hard and brutal.
In the original game only three difficulties of Skirmish AI are able to choose: Easy, Medium, and Brutal. "Extra Hard AI" doesn't represent any additional difficulty level but rather man-made modifications to rules(md).ini and ai(md).ini.
In rules(md).ini you see general rules for engagement including general AI settings;
in ai(md).ini you see the formation and specific behavior of AI's troops.
All the AI settings are stored in these two files since RA2 AI is scripted rather interactive.
What you see on some maps with modded AI is the work of map trigger and ini editing, which overwrites selected sections in rules and in AI from the base game when the match is played on this exclusive map.
So "how", you can either download other's modded AI and rules from the Internet, activate it via CnCNet checkbox, or make your own Brutal AIs that suite your taste.
You can use cncnet or rambo to play the original campaigns too.
