Just FYI for anyone searching. Still haven't found the solution. The game is not in my "crashed" category created specifically for it.

Huh I've encountered this before, but with old games and never with Insurgency. The solution that fixed it for me was going to System Properties->Advanced->Settings tab under Performance->Data Execution Prevention tab->And added an exception for the old games. I have Win 7 premium btw, so IDK if it'll help you. But worth a shot anyway?

DEP is the most probably cause of BEX (Buffer Overflow) error.

Usually DEP prevents using malicious exploits, "insurgency.exe" runs fine for thousands of users so it can hardly be an issue.

This suggest that your System is misconfigured or have corrupted files, so be sure your system is up-to-date and fully functional.

Sorry that it took so long, i'm working with info i've got at the time.

DEP is unavailable on my current system with all and any privileges.
If I recall correctly I forced it off at startup and still got this error.

So basically what I need to do is reinstall my OS to fix the game now.

You are not fixing "the game", it's the other way around.
