You'd probably lose the cosmetics and any remaining Blood Points would just be taken. You'd also likely get blacklisted on Steam for refund abuse so I wouldn't do it. 2 Million Blood Points isn't THAT much big scheme of things and if you're likely to lose the cosmetics when the game isn't in your account anymore there's no reason to bother IMO. I'd just buy it if you're interested in the game and don't if you're not.

They could ban your DBD account if they wanted over that technically.

They -probably- wouldn't, but they could as it violates their tos; and that isn't in steam's jurisdiction really anymore. I've seen other games ban for this kind of thing.

Most likely you would get the bloodpoints because they don't care about that, lose cosmetics when the game is not in your account, and that would just be the end of it.

You could also get your account flagged with steam refunds but tbh only if they saw your thread here; or you admitted outright in the refund reason or something.

The refund itself would constitute abuse of the system, they could be barred from any future refunds on the platform. It's happened before so I wouldn't be shocked if they got banned in DBD and got a Steam Account ban on refunds.

I sincerely doubt they would go into their DbD account and remove the remaining BP. I also doubt anything in DbD would be effected, like having cosmetics taken away. I think the only risk is punishment for abusing the refund system. But I don't have concrete reasons for thinking this, it just feels logical that it wouldn't justify the effort to go into someone's DbD account specifically, and then remove things.
