Graphics settings don't really affect performance. Of the options I know, try:
1. Settings/Video - disable "Enable Reduced Latency".

If that doesn't help, go to Settings/Graphics, and:
2. "Frame Rate Limit" - on, "Custom FPS Limit" - reduce by 5-15 (or more if needed).
3. Change "Resolution Scale" to 85/75% (but this can cause freezes/stutters, so you need to check in real time).
4. Change "Projected Texture Resolution" (half or minimum).

I hope some of this helps.

Most stutters happen from loading things while you are moving the camera around since most items are not rendered until you start looking at them so if you are mid fight or moving too much you start loading and unloading things quickly. Happens a lot in the open world.

Do not run this game on a hardrive. If you have an SSD use it.

And this

My game runs at 150-170 fps without the cap but because its too inconsistent it starts stuttering A LOT so I put the cap at 120fps and the game runs perfectly in every single game mode except when fighting around the open world.

^ This, plus minimising your shadows settings.

In the past I have found that texture sizes and shadows setting are the biggest FPS killers.

It would be wisdom to update your graphics driver if you haven't already done so. Some of the recent Windows updates have been known to cause performance issues with some cards.

Also, you haven't stated what display resolution you are trying to run at, but that old card will likely struggle to produce a reasonable frame rate at anything greater than 2560 x 1440.
