Thats when the game begins.
I would recommend playing through the base game before skipping to the DLC. The advice to skipping to NY instantly is terrible advice.
So after you beat teh story, you start World Tiers (loot gets stronger with enemies). However, you can do NY which starts at 30 till 40. Which creates a whole new world of loot tier. @40 no longer tiers but just the level itself. After i finished the main story I just skipped the tiers and went straight to NY. Just finished last night. So far, a lot of fun.
Except the last boss fight... that was dumb.
Tier 5 thing was prior to NY DLC. Now it is moot. Going to Tier 5 just delays you to get the max level of 40 and start working on end game. Plus you get gear that makes Tier 5 look weak, at start of NY
