The shrines can be found by following the blood trail that leaves the lost page altar. Though you don't have to find this. Begin following the blood trail anywhere you see it.
Once you reach the end of the trail, you'll see one of two shrines. Approach the shrine and it will activate. Once activated you will need to kill a certain numbers of enemies inside the purple circle(s) around the shrine. The number needed is displayed above the shrine. For the 3-spread shrine each circle needs a similar number of kills and will disappear once you have completed each circle.

Upon completing your first shrine, pickup the token. It should appear next to your EXP meter (The purple token below the Gatherer's Charm in the screenshots). Follow the blood trail in reverse to find the next shrine and collect the second piece.

First step is to locate the Ember Eye. This can be done by following the statues beginning from the lost page alter. You do not need to start at the altar, similar to Haunted Caverns, you may begin following the statues from the first you see. To follow the statues go in the direction their hands point.

Upon pickup this will appear as an icon next to your EXP meter. You should now follow the white iris effect on the icon to find the hidden boss. Note that the iris effect grows dimmer the further away you are from the hidden boss. It can be VERY faint. (in the screenshot you see it facing East and slightly down)
The hidden boss will first appear as a pile of rubble. Once you approach (with the Ember Eye), it will activate and the hidden boss will spawn. The spawn location for the rubble is random.

Once you defeat the boss it will drop the Cracked Ember Eye. Pick this up to complete the event. It will appear next to your EXP meter.

Normally, these bombs will chase the player and detonate when nearby. They deal damage in an AOE (base 10 damage) and disappear afterwards.
Completing the quest removes the damage done by the lava bombs. It also changes their appearance to be orange. Additionally, lava bombs now randomly last up to 3s longer and detonate 15% further away from the player. Not that this matters, because they no longer deal any damage but it does look a lot cooler!

Fortunately, the raven will periodically cast "Raven Wings" in AOE around it. This grants the player a +40% movement speed buff. This buff appears in the players buff list (with the same icon as the speed power-up) and will also show in any tooltips for movement speed. You do not need to keep up with the raven. It will periodically wait along the route before casting Raven Wings when you approach and then flying off again.

To reach the sarcophagus you will have to pass through the ghost wall (or kill it and then pass through it). Keep this in mind. To pass through the ghost wall you should either have a high movement speed, or a balanced HP/def combo. You can expect to take approximately 3-4 hits of about 80 damage.

Once you reach the sarcophagus you must destroy it. It has 30,000 base HP and has 50% damage reduction. Once destroyed, collect your Sentinel Orb to complete the quest. The token will appear next to your EXP bar.

The boss will first appear as a shredded cloth on the ground. Approach this to activate the hidden boss.
Defeat the boss and pickup the Hating Heart to complete the quest. This appears as an icon next to your EXP bar.

Firstly, it will change the frequency of attack patterns. The boss will now be +100% more likely to perform a spin attack, and 33% less likely to do the jump attack.
Secondly, after the boss performs a spin attack it will become "winded". This debuffs the boss with -30 defense for 3s afterwards (boss still retains the +40 defense during the spin attack). This is telegraphed to the player by the hating heart icon pulsing. It will pulse once at the start of the debuff. Additionally, the boss will also remain stationary in a "winded" position during the debuff. This debuff is approximately equal to the player doing double damage, over and above all other existing buffs.

Beginning at the lost page altar find the first active device. It should be nearby in one of the NW/NE/SE/SW directions. This device is denoted by the light ray coming from it. Follow the light ray to the end. If the light ray is hitting another light bulb, then that light bulb should have lit up and produced another light ray. Continue following.
Once you find a light ray that ends (maximum range having not hit anything) or hits a wall, return to the dissonator and rotate it. To rotate the device, walk into melee range and touch it. These devices rotate 90 degrees counter-clockwise each time you touch them. Follow the new light ray. If it has not hit a new dissonator, return and repeat until it does.
If you find a light bulb where all 3 directions end without hitting another, then backtrack to the previous light bulb and rotate it until it hits a new light bulb. It is possible for one light bulb have multiple other light bulbs in range. However, there is only one correct route to complete the quest. By backtracking to a previous light bulb and beginning the process again it will eventually complete the quest. This whole process can be completed in a minute or may take several minutes. It all depends on your route generation (luck).
To complete the quest, simply have the final light bulb hit the Dissonator Monolith. The device should light up yellow and have intermittent lightning effects. It will also spawn a large AOE circle around itself (quite hard to see, visible in screenshot as small yellow lights going up/down from the only damage number).

If the final boss is not within range, then it will probably* attack the highest HP enemy. It will deal 1,000 base damage and then apply 1 base fragile stack.
If the final boss is within range, then it will only attack the final boss. It will deal 7,000 base damage and then apply 2 base fragile stacks.
* The game locates the highest HP mob by finding an enemy with more max HP than the current one. It checks all enemies within range, adding them to a list each time it finds a new one with more max HP. It then selects one at random from this list. The list can be, at most, equal to the number of unique waves currently spawning. So, strictly speaking, it selects several mobs of increasing HP and then chooses one at random. This isn't really attacking the highest HP mob but I didn't know how else to phrase it.
The only one I find difficult is the lord of the first area, which already had this kind of mechanic since the start.
